and attach a copy of the following documents:

- Valid Italian passport for every member of the family;
- Document allowing you to reside in the United States (long term visa, Permanent Resident Card, US or Canadian passport, certificate of naturalization if you are a dual citizen);
- Proof of residence (driving licence, electric or phone bils etc).
