Declaration for minors under the age of 14 travelling alone or with people other than their parents

In order to travel alone or with people other than their parents, Italian citizens under the age of 14 must possess a Declaration issued by the Consulate. The declaration is free of charge and is issued by the Consulate upon request by the parents for all those minors resident in the jurisdiction of the Consulate General of New York. The Declaration can be requested by filling out this form (link to the form) and by providing the following documentation:

· copy of the parents passport

· copy of the minor’s passport

· copy of the accompanying adult’s passport

· airline and flight information

The declaration can be requested during Office hour (Mon – Fri, 9:00 – 12:30 except Wed) without the need to book an appointment. It is suggested to submit the requested documents in advance to the Office by e-mail (newyork.pass@esteri.it) or fax (212) 439 8674, in order to accelerate the process.

Please note that if you are a non-European citizen parent, your signature must be notarized (at any Notary Public) or you must sign the form in front of a Consular Attaché.
